Hours of Operation Thursday -Sunday 9am - 7:30pm
Duties and Responsibilities
- Coordinate the care for the patient/family under the direction of the primary physician and registered nurse.
- Responsible for identifying patient/family needs.
- Coordinating the care for the patient/family.
- As part of the interdisciplinary group (IDG) contributes and follows an individualized plan of care
- Utilizes monitoring, intervention and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.
- Pledge to make the comfort and care of patients and families the highest priority
- Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times
- Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.
Hours of Operation: Mon - Fri 7:00a - 3:30p and every other weekend
Duties and Responsibilities
- Provides direct and indirect patient care in an inpatient or outpatient setting under the direction of a Registered Nurse or Licensed Practical Nurse.
- Must assist residents with activities of daily living, provide for personal care, comfort, and assists in the maintenance of a safe and clean environment for an assigned group of inpatient residents.
- Must also provide and maintain bodily and emotional comfort to Hospice patients/families and to educate and assist the patient/family in maintaining the maximum level of independent living in a safe environment.

- Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times.
- Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.
Hours of Operations: M-F: 8:30am - 5pm, three days per week; and 11:30 - 8pm, two days per week. Occasional weekend or evening events
- Implements the established bereavement support services and protocols through client assessment, goal setting, plan of care implementation, progress evaluation, documentation, advocacy, and coordination of service delivery until client goal resolution is achieved.
- Clients are defined as adults and children. Provides short term bereavement counseling through emotional and psychosocial support and services to assist with issues related to grief, loss, and adjustment.
Hours of operation: Monday - Friday - 6:30am - 3pm and every other weekend
