Duties and Responsibilities
- Carries primary responsibility for evaluation of all patients referred for hospice services.
- Is able to articulate all components of the organization's continuum of care and refer patients and families to those as necessary.
- Assists referring physicians and/or his/her designated liaison with coordinating a plan of care/treatment for hospice appropriate patients and their families.
- Serves as the liaison between our company and physicians, hospitals, long-term care facilities, and other health care professionals.
- Actively markets, through education, the hospice concept to all health care professionals and providers, as well as the community at large.

- Must also provide and maintain bodily and emotional comfort to Hospice patients/families and to educate and assist the patient/family in maintaining the maximum level of independent living in a safe environment.

- The CareConnection Coordinator is responsible for coordinating with CareConnection Managers and team members in patient/family direct and indirect care needs and provides quality professional intervention based on the protocols and policy with regard to the physical, emotional and psychological needs.
- Interacts with management, physicians and field staff utilizing a teamwork approach.
- Serves as a professional leader in assessing, planning, guiding and evaluating patient care activity.
- Provides support and assists in training and evaluation of all Care Connection clinical and non-clinical staff, according to position qualifications and standards of practice.

- The Billing & Collections Specialist is primarily responsible for patient billing, collections and accounts receivable activity for all payor sources, in accordance with established policies and procedures.
- The Billing & Collections Specialist is responsible for account follow-up, resolving billing problems and answering patient inquiries.
- Sets up financial arrangements as needed.
- Uses collection techniques to keep accounts receivable current including monitoring for delinquent payments.
- Has training and experience in Medicare, Medicaid and commercial insurance billing and experience in various software systems, including Electronic Medical Records.

- Provides pick-up and delivery services for all DME and oxygen equipment to patients and families.
- Instructs clients as to the proper use of equipment.
- Responsible for the final inspection of equipment prior to delivery.
- Completes home safety assessment relating to patients environment for safety purposes.
- Maintains quality compliance and adheres to all infection control practices and policies.
- This position involves direct contact with patients and families so must communicate effectively and provide a high level of customer service.

JOB SUMMARY
- Functions as a problem solver and facilitator with the principle objective of providing continuity of care to meet the needs of patients and families.
- As a partner, primary role is to serve on the Admission team as a Rapid Responder obtaining signed consents, processing Pending Referrals and pending list phone calls, completing non-nursing documentation in any healthcare setting, assisting with the admissions process from Intake to the Field and responding to patient needs by representing the Trustbridge values.
