JOB SUMMARY
- The Master's level Social Worker is responsible for assessment and intervention with the patient and family within his or her scope of practice as a member of the interdisciplinary team.
- Performs duties that include assessment, intervention, care planning, outcome evaluation, documentation and collaboration/communication with the Interdisciplinary Team to assist patients and families with their goals and assist the team in the delivery of hospice care to the patient and family.
- May supervise interns and unlicensed social workers.
JOB SUMMARY
The Associate Medical Director is a TrustBridge, Inc. employee responsible for the direction of the care and treatment of patients served by the Hospice Care Groups, in cooperation with the patient’s attending physician. The Associate Medical Director is responsible for determining prognosis and eligibility for hospice services, as well as providing medical guidance to the interdisciplinary Care Group employees and volunteers in the management of the patient's plan of care. He/she strives to ensure excellent care and service as evidenced by AIM measures, patient/family satisfaction survey, and/or other quality indicators as defined by the organization. He/she serves as liaison between the organization and both the medical community and the community-at-large. He/she acts as an ambassador and an advocate for hospice services, as well as all services in the TrustBridge continuum of care.
Hours of operation: 8:30am - 5pm and weekend requirement

JOB SUMMARY
- Functions as a problem solver and facilitator with the principle objective of providing continuity of care to meet the needs of patients and families.
- As a partner, primary role is to serve on the Admission team as a Rapid Responder obtaining signed consents, processing Pending Referrals and pending list phone calls, completing non-nursing documentation in any healthcare setting, assisting with the admissions process from Intake to the Field and responding to patient needs by representing the Trustbridge values.
- Assists with coordinating the plan of care and treatment (in person or telephonic) for hospice appropriate patients and their families.
- Able to articulate all components of the organization's continuum of care, guiding patients and families to necessary services.

- Provides direct and indirect patient care in an inpatient or outpatient setting under the direction of a Registered Nurse or Licensed Practical Nurse.
- Must assist residents with activities of daily living, provide for personal care, comfort, and assists in the maintenance of a safe and clean environment for an assigned group of inpatient residents.
- Must also provide and maintain bodily and emotional comfort to Hospice patients/families and to educate and assist the patient/family in maintaining the maximum level of independent living in a safe environment.

- Provides pick-up and delivery services for all DME and oxygen equipment to patients and families.
- Instructs clients as to the proper use of equipment.
- Responsible for the final inspection of equipment prior to delivery.
- Completes home safety assessment relating to patients environment for safety purposes.
- Maintains quality compliance and adheres to all infection control practices and policies.
- This position involves direct contact with patients and families so must communicate effectively and provide a high level of customer service.
