Job Summary

JOB SUMMARY
- The Associate Medical Director is a TrustBridge, Inc. employee responsible for the direction of the care and treatment of patients served by the Hospice Care Groups, in cooperation with the patient’s attending physician.
- The Associate Medical Director is responsible for determining prognosis and eligibility for hospice services, as well as providing medical guidance to the interdisciplinary Care Group employees and volunteers in the management of the patient's plan of care.
- He/she strives to ensure excellent care and service as evidenced by AIM measures, patient/family satisfaction survey, and/or other quality indicators as defined by the organization.
- He/she serves as liaison between the organization and both the medical community and the community-at-large.
- He/she acts as an ambassador and an advocate for hospice services, as well as all services in the TrustBridge continuum of care.
Hours of operation: 9am - 7:30pm and every other weekend
Duties and Responsibilities
- Carries primary responsibility for evaluation of all patients referred for hospice services.
- Is able to articulate all components of the organization's continuum of care and refer patients and families to those as necessary.
- Assists referring physicians and/or his/her designated liaison with coordinating a plan of care/treatment for hospice appropriate patients and their families.
- Serves as the liaison between our company and physicians, hospitals, long-term care facilities, and other health care professionals.
- Actively markets, through education, the hospice concept to all health care professionals and providers, as well as the community at large.
Hours of operation: 1 day: 9am - 7:30pm and weekend availability

The function of the IT Help Desk Support Specialist is to provide first level support to the organizations users via phone and Email, as well as adding, updating and monitoring Help Desk tickets utilizing the Help Desk system. This is a support position, mainly providing telephone assistance.

JOB SUMMARY
- Functions as a problem solver and facilitator with the principle objective of providing continuity of care to meet the needs of patients and families.
- As a partner, primary role is to serve on the Admission team as a Rapid Responder obtaining signed consents, processing Pending Referrals and pending list phone calls, completing non-nursing documentation in any healthcare setting, assisting with the admissions process from Intake to the Field and responding to patient needs by representing the Trustbridge values.

- The Scheduling Coordinator position is the key position to coordinate staffing and provides timely and appropriate scheduling for safe and effective patient care delivery.
- Optimally schedules RNs, CNAs, LPNs, Medical Social Workers, Integrative Therapists and Chaplains to accomplish patient visits in accordance with the patient’s plan of care.
- Maintains coordination of the Weekend/Holiday Staff Schedule, the On-Call RN schedule and the daily assignments.
- Ensures that communications from patients, physicians and caregivers are relayed to clinicians in a timely manner.
- Responsible for assigning and verifying visits for accuracy to facilitate accurate payroll and billing processes.
- Demonstrates effective liaison activities and interdepartmental communication and cooperation to support essential departmental activities.
- This position will require weekend rotation.
