Overview

Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and customer-focused Admissions Coordinator to join our team in West Palm Beach.

 

There are several full-time opportunities available which support the admissions process and ensure a seamless, empathetic experience for patients and families during one of life’s most meaningful transitions. The available shifts are:

 

  • Monday through Thursday, 7:30 a.m. – 6:00 p.m.
  • Monday through Friday,11:00 a.m. – 7:30 p.m
  • Monday through Friday, 12:00 p.m. – 8:30 p.m

 

Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

 

 

 

 

Responsibilities

What You’ll Do

  • Facilitate the conversion of referrals to admissions through completion of the intake process.
  • Provide exceptional customer service to patients, families, and referral sources with empathy and professionalism.
  • Communicate effectively with interdisciplinary team members to ensure timely and accurate admissions.
  • Maintain accurate records and documentation in accordance with organizational policies.
  • Support a compassionate, patient-centered environment that reflects the mission and values of Trustbridge Hospice.

What You’ll Need

  • Associate degree preferred.
  • Three (3) years of experience in healthcare administration or customer service.
  • Knowledge of hospice or home health programs preferred, with willingness to develop clinical understanding.
  • Ability to handle a high call volume.
  • Strong communication, organization, and computer skills.
  • Bilingual (English/Spanish) preferred.
