Overview

Pay range:  $23/hr. - $29/hr. based on experience

Hospice nurses provide end of life care to help ensure the comfort and quality of life for their patients.  They primarily work in collaborative, team-driven environments that require frequent communication with their patients, families and other caregivers.  The ideal candidate should be detail-oriented, patient and calm.  Hospice nurses are caring and kind individuals who possess a special hospice heart who can help our patients celebrate the rest of their life’s journey.  

Responsibilities

Hours of operation:  Fri/Sat/Sun 6am - 6:30pm

Work location:  West Palm Beach

  • The CareConnection Coordinator is responsible for coordinating with CareConnection Managers and team members in patient/family direct and indirect care needs and provides quality professional intervention based on the protocols and policy with regard to the physical, emotional and psychological needs.
  • Interacts with management, physicians and field staff utilizing a teamwork approach.
  • Serves as a professional leader in assessing, planning, guiding and evaluating patient care activity.
  • Provides support and assists in training and evaluation of all Care Connection clinical and non-clinical staff, according to position qualifications and standards of practice.

 

 

Qualifications

 

  1. Education/Regulatory Requirements:
    • Current license to practice as a Licensed Practical Nurse in the State of Florida.
    • Current Basic Life Support Certification.
    • 30 hour IV Certification preferred.
    • Hospice Certification preferred.
  2. Skills:
    • Minimum experience of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing.
    • Understanding of the ability to apply the characteristics of hospice nursing, case management, crisis intervention, communication skills, thanatology skills and needs.
    • Basic computer skills and EMR software experience a plus.
    • Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
  3. Professional Requirements:
    • Able to communicate effectively with other Hospice care personnel. 
    • Able to coordinate with other health care providers in the delivery of patient care.
    • Able to work effectively with lay and professional volunteers.
  4. Language Skills:
    • Bilingual (English/Spanish) preferred.
