Use your nursing skills to make a difference. Hospice nurses provide end of life care to help ensure the comfort and quality of life for their patients.  They primarily work in collaborative, team-driven environments that require frequent communication with their patients, families and other caregivers.  The ideal candidate should be detail-oriented, patient and calm.  Hospice nurses are caring and kind individuals who possess a special hospice heart who can help our patients celebrate the rest of their life’s journey.  

Responsibilities

-Assists the Director of Admissions in the administrative management of the Admissions Department making decisions as outlined by established standards, policies and procedures.
-Provides professional leadership to the Admissions Department, its Team members and the community at large.
-Serves as liaison between the Admissions Department and internal partners in all areas of the organization.

-Works in conjunction with the Regional Access Director to meet goals of conversion with referrals, staff productivity, quality patient care and compliance standards.

-Manages the daily evening operations including staffing during evening hours or as designated responsibilities.

-Coordinates patient referrals and assigns visits to admission staff to ensure an effective and efficient admission.

-Assists in the processing, monitoring and conversion of referrals for Hospice services and may include completing Explanation of Service, contacting individual patient, family or referral sources.

-Provides clinical support and collaborates effectively with Admission RN’s, hospital team, resource physicians, and other Trustbridge Hospice Teams and community physicians regarding admission criteria, care planning and active treatment authorization.

-Initiates the coordination of complex cases including inpatient and collaborative visits.

-Ensures staff accurately enters all admission data/orders/care plan into the electronic medical record (EMR).

-Appropriately and confidentially handles high-level, information as a regular part of daily operations.

-Achieves the established strategic goals pertaining to NTUC admissions, conversion and average daily census; ensures continuity within Trustbridge programs. Reports high risk or problematic situations.

-Functions as an effective team member who uses communication and leadership strategies, time management, effective problem solving and an awareness of ethical and cultural issues while engaged in the referral and admission process for patients and families.

-Arranges complex care conference calls with receiving team to effectively transition care.

-Collaborates effectively with Business Development, Access Center and Admissions team.

-Serves as the primary contact and coordinator for hospital case management, staff, physicians, other Trustbridge Teams, and palliative supportive care staff.

-Available to complete admissions as needed.  

-Manages, coaches and disciplines staff to assure Trustbridge Code of Conduct and Mission is followed.

-Assist with customer recovery if needed for all parties involved
-Availability during the hours of: 8:30am-5pm and beyond, when needed.
-Position is located in Boca Raton.


Pay range:  $108k - $135K based on experience

 

Qualifications

-RN degree from a college or university, accredited by the Accreditation Commission for Education in Nursing and/or the Commission on Collegiate Nursing Education.

-Florida licensed RN.

-BSN preferred.

-One year experience in a leadership role or demonstrated capability of same required.

-Computer literate.

-Flexible, creative, assertive, articulate, nurturing and self-nurturing.

-Homecare Homebase software experience a plus.

-Effective oral and written communication skills.

-Ability to develop and apply effective customer service skills.

-Demonstrates organizational, administrative and personnel management skills.

-Ability to work well under stress and with a sense of urgency to meet deadlines.
-Bilingual (English/Spanish) preferred.
