Responsibilities

Hours of operation: 3 days - 8:30am - 5pm & 2 days - 11am - 7:30pm

  • The Grief Care Counselor will provide community-based individual, family and group grief counseling, education, crisis interventions, and support services to clients in the service area not cared for in the hospice division.
  • This position will also provide grief workshops/seminars, grief support programming, resource information and referrals to other agencies as appropriate.
  • This position is primarily office based and provides community grief support services to children, teens, adults, and families.
  • Provides short term brief focused individual, group, and family grief counseling services to children, teens, adults, and families.
  • Conducts intake assessments of children and families to determine eligibility for grief support services and group readiness
  • Assesses need for counseling related to risk assessment for pathological grief, trauma informed care, risk of suicide, abuse, or neglect
  • Mandated reporter in the event of child, adult or elder abuse.
  • Facilitates support groups for children, adults and families on an ongoing basis to meet community’s bereavement needs
  • Provides on-going case management, goal setting, re-assessment and care plan updates for individual children, adults, and families
  • Assists with special events, outings, crisis interventions, as well as community based education and support events for teens, adults and families
  • Assists with curriculum planning for grief support groups at all locations
  • Maintains documentation within the Electronic Medical Record (EMR), including written Plan of Care, and ensures that services provided, and contact made are documented daily and follow all organization and regulatory requirements as required for surveying and accrediting agencies.
  • Educates colleagues, volunteers, facilities, and community members regarding the variety of services available
  • Responsible for complete confidentiality of client records
  • Attend all counseling center meetings and other meetings as requested.
  • Develop and facilitate support groups that support the needs of the counseling center clients.
  • Perform other duties as assigned by the Grief Care Manager or Senior Vice President of Community Services.
  • Experienced with providing individual, group and family counseling services to all age groups.
  • Excellent clinical counseling skills with the ability to build rapport and implement age appropriate counseling interventions.
  • Ability to facilitate community educational and grief support presentations
  • Ability to speak effectively before groups of customers or colleagues.
  • Excellent organizational skills with the ability to work independently and as part of a team.
  • Demonstrated proficiency in Microsoft Windows, Office Suite, and Empath Health related software programs and keyboarding skills (35wpm).
  • Ability to communicate and establish effective working relationships, utilizing problem-solving techniques.
  • Ability to anticipate and handle any bereavement situation, including crisis interventions
  • Knowledgeable and able to speak comfortably regarding the stages of dying and post death bereavement and the grieving process with patients, care givers, colleagues, and community.
  • Must be able to travel within Empath Health’s service areas as needed during daylight and night time hours in all weather conditions and abide by organizational motor vehicle record (MVR) guidelines.
  • Must be willing to work evening and weekend hours to accommodate children and family schedules.

 

Qualifications

 

Education/Regulatory Requirements:

  • Master of Social Work (MSW) degree from a school of Social Work accredited by the Council on Social Work Education preferred; or Master’s degree from a clinical counseling program such as mental health or marriage and family therapy.
  • Required to have one of the following credentials:
  • Licensed in the State of Florida in one of the following
    • Social Work (LCSW), Mental Health Counselor (LMHC), Marriage and Family Therapist (LMFT), or has applied for one of the above licenses and will be licensed in the State of Florida within 90 days of hire.
  • May also be:
  • License eligible (Registered Intern) in the State of Florida in one of the following:
  • Social Work (RCSWI) or
  • Mental Health Counselor (RMHCI) or
  • Marriage and Family Therapist (RMFTI) or
  • Has applied for one of the above licenses and will be licensed as a registered intern in the State of Florida within 90 days of hire

Skills:

  • At least two years of counseling experience working with children, teens, adults and families.
  • At least one year of social work/counseling experience in a healthcare setting (the one-year Masters level internship would meet this requirement).
  • The ability to type 25 wpm is highly preferred. Must receive a passing grade on a pre-employment computer literacy evaluation.
  • Previous experience in the strategic design and implementation of human service programs and proven ability to manage these programs for quality and cost effectiveness preferred
  • Previous experience in grant procurement and management preferred.
