Overview

Pay range:  $30/hr. - $40/hr. based on experience

Hospice nurses provide end of life care to help ensure the comfort and quality of life for their patients.  They primarily work in collaborative, team-driven environments that require frequent communication with their patients, families and other caregivers.  The ideal candidate should be detail-oriented, patient and calm.  Hospice nurses are caring and kind individuals who possess a special hospice heart who can help our patients celebrate the rest of their life’s journey.  

Responsibilities

Hours of operation:  9am - 7:30pm occassional weekends

  1. Carries primary responsibility for evaluation of all patients referred for hospice services.
  2. Is able to articulate all components of the organization's continuum of care and refer patients and families to those as necessary.
  3. Assists referring physicians and/or his/her designated liaison with coordinating a plan of care/treatment for hospice appropriate patients and their families.
  4. Serves as the liaison between our company and physicians, hospitals, long-term care facilities, and other health care professionals.
  5. Actively markets, through education, the hospice concept to all health care professionals and providers, as well as the community at large.
  6. The Lead Admission RN will train, teach, educate, mentor new and identified AN team members who require guidance and support.
  7. Formulate and organize training materials to assist in streamlining admission process and education for AN team members.
  8. Assist in establishing standard, clear, concise CTI presentation application with all AN Team members.
  9. Instrumental in promoting and teaching strong clinical knowledge to AN team, resulting in admission department reaching high patient conversion goals.

Qualifications

 

  1. Education/Regulatory Requirements:
    • Registered Nurse currently licensed in the State of Florida.
    • BSN preferred
    • Hospice certification preferred.
    • Current Basic Life Support Certification preferred
  2. Skills:
    • Must possess ability to assess physical and psychosocial needs of patient and family.
    • Hospice experience highly desirable.
    • Organized, flexible, self motivated with ability to relate to all levels of the professional medical team.
    • Ability to develop and apply effective customer service skills
    • Able to work independently with minimum supervision.

  3. Professional Requirements:
    • Demonstrates excellent verbal and written communication skills.
    • Electronic medical record software experience a plus.
  4. Language Skills:
    • Bilingual (English/Spanish) preferred.
