Pay range:  $30/hr. - $40/hr. based on experience

Hospice nurses provide end of life care to help ensure the comfort and quality of life for their patients.  They primarily work in collaborative, team-driven environments that require frequent communication with their patients, families and other caregivers.  The ideal candidate should be detail-oriented, patient and calm.  Hospice nurses are caring and kind individuals who possess a special hospice heart who can help our patients celebrate the rest of their life’s journey.  

Hours of operation:  12pm - 10:30pm and every other weekend

Duties and Responsibilities

• Carries primary responsibility for evaluation of all patients referred for hospice services.
• Is able to articulate all components of the organization's continuum of care and refer patients and families to those as necessary.
• Assists referring physicians and/or his/her designated liaison with coordinating a plan of care/treatment for hospice appropriate patients and their families.
• Serves as the liaison between our company and physicians, hospitals, long-term care facilities, and other health care professionals.
• Actively markets, through education, the hospice concept to all health care professionals and providers, as well as the community at large.

A. Education/ Regulatory Requirements:

• Florida licensed R.N.; B.S.N. preferred.
• 1 year medical surgical experience preferred

B. Skills:

• Preferred CHPN Certification.
• Must possess ability to assess physical and psychosocial needs of patient and family.  Hospice experience highly desirable.
• Demonstrates excellent verbal and written communication skills.  Bilingual Preferred. Electronic medical record software experience a plus.
• Organized, flexible, self motivated with ability to relate to all levels of the professional medical team.
• Ability to develop and apply effective customer service skills.  Able to work independently with minimum supervision.
